apport package hook only run for mysql-server-5.1

Bug Description
The way the apport package hook currently works it is only run when some executes 'ubuntu-bug mysql-server-5.1', however I'd imagine it should also be ran when someone runs 'ubuntu-bug mysql-client' or 'ubuntu-bug mysql-common'.
The way to make this happen is to install the hook so it named after the source package - mysql-dfsg-5.1. That way ubuntu-bug with any binary package name will have the hook run.
