team emails do not set headers right

Bug Description

The team related emails (E.g. foo person waiting for approval to join team) fail to set the X-Generated-By: launchpad (canonical.com) header.

It'd be nice if they did.

Perhaps we should make simple_sendmail automatically add this header. Are there situations when we *don't* want to add this header?

simple_sendmail calls sendmail, which already does add the X-Generated-By header, so these emails must be generated through a different code path.

Brad's right: this points to a problem where we're generating emails in a non-standard way which could cause all sorts of problems.
