When trying to add a domain, I noticed that the form does not correctly align with the design.
[Reproduce]
1. Go to DNS page/tab.
2. Click on 'Add Domain'
3. You will see this http://
If you see at 3), the form is misplaced and at the same level of the 'Add XXX' button. Similar to bug #1587936
