Bug Description

Backport the below upstream "powernv-cpufreq" bug fixes to ubuntu 16.04:

The below listed patchset is accepted upstream and is in linux-next.

4) 0306e481d479a58eff17c27adf213fbb5822946b cpufreq: powernv/tracing: Add powernv_throttle tracepoint
5)c89f2682a39192433c296bf97b834fd2815a758b cpufreq: powernv: Replace pr_info with trace print for throttle event

1) Fixes a memory leak
2) Makes the worker thread hot-plug safe which is scheduled on an OCC event.
3) Fixes the below bug which cause 4% cpu overhead by powernv_cpufreq_throttle_check() function.

I cherry-picked the first 5 patches from linux-next, but I'm not having much luck scraping 6 and 7 from marc.info.
