no warning upon headset plugin

Are you sure the *headset* volume is maximum? The speaker volume and headset volume are independent settings, so setting one to max won't alter the other.

They are different values, so it could definitely be that your headset volume is not at a warning level.

But, the flip side is should we always show a notification in the case that we plug in a headset? It seems that might be useful as feedback that the audio state has changed.

Yes, plugging in the headset should show a notification, regardless of whether volume is high or not. If it happens to be high, it will be the warning version.
