How exactly did you call ubuntu-bug? This output is interesting:

Here's what i did and it can be reproducible easily.

This happens when you press Cancel button when prompted to enter the password to collect information to be included in the bug.

So basically
1. Ubuntu-bug ubiquity
2. Cancel when it prompts to enter the password

** Occurs in precise as well
