Don't account for time in throttled state.

When we get throttled, we set a timer to be able to remove the time
where we are just waiting throttled. This makes timing metrics more
accurate.

Fix how current tps is computed.

With a low current TPS, the division will lead to a value of 0 TPS. We
multiply first, then divide to avoid this. This is a regression
introduced by fixing LP #613342. We also cast to uint64_t earlier.
