Updated README.ex sample readme. Removed some debugging stuff from rest-relation-changed hook.

Added a filters config option that is really required since it allows setting up the filter section of the logstash config file. Get password from ampq and use it to update the logstash config.

Fix amqp relation changed. Added amqp-relation-joined so that logstash-indexer can set relation parameters username and vhost which are expected by rabbitmq-server.

Use rest-relation-changed instead of rest-relation-joined because that's where the action is. Use #juju-elasticsearch-host and #juju-amqp-host as markers for the lines we need to update in logstash config.
